Patient Falls With Injury

What are we looking to improve?
Falls are known to be one of the most common adverse events for patients during their hospital stay. Falls that result in injury can actually increase patientsí length of stay and increase their risk of morbidity and mortality; this is especially true for elderly patients. According to national data, falls during hospitalization are on the rise, placing patients at risk. By following Core Measures, we look to prevent falls.

What measures/strategies have we implemented for improvement?

How do we score?
During the 2nd quarter 2012, Mount Sinai St. Luke's Roosevelt had three falls that resulted in injury, for a rate of 0.06% based on hospital discharges.
