The deadline for 2009 PASS applications is October 19. Program Requirements:

• Three (3) letters of recommendation with at least one from an oral & maxillofacial surgeon
• Attributes such as ability to work in a hospital environment, initiative, responsibility, cooperativeness, and strong work ethic
• Pass Parts I & II of the National Dental Board Examination
• Citizen and/or permanent resident of United States or Canada
• Graduate of U.S. or Canadian dental school
• Proficient in English (the ability to speak Spanish is beneficial)
• Submission of 2"x 2" photograph directly to program
• License or limited permit to work in the state of New York
