The Standardized Infection Ratio (SIR) is the new term utilized by Medicare and the Joint Commission to monitor surgical site infection. SIR measures institutional infection rates compared to national standards. The ratio is defined as the hospital infection rate divided by the risk adjusted national infection rate as determined by the NHSN. An SIR <1.0 indcates that the hospital infection rate is better than the national standard.
