Scheduling an appointment with the Cardiovascular Disease Prevention Program at St. Luke's and Roosevelt Hospitals is the first step in achieving your goal of lowering your risk for cardiovascular disease. The Cardiovascular Disease Prevention Program is focused on reducing risk for heart attacks, stroke, and peripheral vascular disease and our staff will work with you to ensure your achieve your goals.

The day of your first visit please bring the following:

• Completed referral form
• Completed health questionnaire ("Intake Form")
• Copies of your latest blood tests any other health information
• Names and addresses of your other health care providers.

We accept Medicare and many insurance plans. Please confirm ahead of time that your plan is accepted.
