Before Your First Visit

The Cardiovascular Disease Prevention Program welcomes patients for either a one-time consultation or for ongoing counseling and treatment. We take referrals from other health care providers but you may also make your own appointment to see us.
After each visit, you will receive a written summary and treatment plan. We will coordinate plans with a primary provider or specialist if you are already receiving medical care.
People who may benefit from our program include those with one or more risk factors, a family history of cardiovascular disease, diabetes, have just had bypass surgery or a stent placed, or suffered a stroke. We have assembled a team of experts ready to help.
