It is with great pleasure that we announce to you the formation of the Continuum Provider Partners IPA, LLC.

This new independent practice organization is being formed for the purpose of promoting collaboration across all the facilities and with all affiliated providers, both employed and voluntary, of Continuum Health Partners, Inc.

This comprehensive network of providers will be equipped with the resources and have the critical mass to manage and improve quality of care for populations as well as individuals through improved access, coordinated care, and improved efficiency. It will be able to demonstrate value to payers and will also have the potential to create economic benefits for all parties.
